
The following will briefly outline some of the things you may wish to know about
my practice. Please know that I welcome any and all questions that you may have.
Appointments:
Appointments are made between 8:00 a.m. - 6:00 p.m. Tuesday and Wednesday. Appointments may be scheduled by phoning 651-319-9269.
Please provide at least
24 hours notice if you need to cancel your appointment. All sessions are 50
minutes unless another time frame is arranged before the session.
Fees & Payments:
Each client is expected to make full payment for all charges at the time of
service. I will be happy to provide you with a receipt if you wish to submit to
your insurance company for reimbursement. Please check with your customer
service insurance representative. Many offer “out-of network benefits” I am a
BlueCross BlueShield Provider. I am not a Medicare Provider.
Intial assessment is $175.00 and additional sessions are $120.00.
Phone Calls and Messages:
Calls are returned within 24 hours however; messages are not checked on
weekends.
Emergency Care:
I do not provide emergency care and if you feel your situation requires immediate
attention you should call:

• Harriet Tubman Battered Women’s Shelter 612- 825-3333
• Go to your nearest hospital emergency room
• Call 911

Therapy involves a confidential relationship. No one will be provided with
information about you without your consent. The exceptions to this are the ones
that Psychologists are mandated to conform to by Minnesota State Law. This law
requires that a verbal or written report be made to either the County Welfare
Department or the Police Department when a psychologist has knowledge or
reasonable cause to believe that:
• A child is being neglected or physically or sexually abused.
• A vulnerable adult is being mistreated.
• Physical harm may be inflicted on someone else or on oneself.
Finally, if you are involved in a court action, your records may be subpoenaed.
