

• The Client has a right to reasonable notice of changes in
services or charges.
• The client has the right to courteous treatment and to be free
from being the object of any discrimination based on race, religion,
gender, or any other unlawful category while receiving psychotherapy
services.
• The client has the right to complete and current information
concerning the clinician’s assessment, recommended course of
treatment, including the expected duration of treatment.
• Client records are confidential, unless release of these records is
authorized in writing by the client or otherwise provided by law.
• The client may refuse services or treatment, unless otherwise
provided by law.
• The client has a right to a coordinated transfer when there will
be a change in the provider of services.
• The client has the right to examine public records maintained by
the Minnesota Board of Psychology, which contain the credentials of
the psychologist.
• The client has the right to expect the clinician has met minimum
qualifications in training and experience required by state law.
• The client may assert their rights without retaliation.
• Grievance: If, for any reason you are dissatisfied with the
service I provide, please talk it over with me to see if we can resolve
it. If you believe your rights have been violated, you may contact the
Minnesota Board of Psychology.
